Founded in 1998, Ultimate Home Health Care offers compassionate in-home medical care for sick, disabled, and elderly adults in and near Chicago, Illinois. We watch over all persons without regard to race, color, national origin, disability, or age.

Medicare provides a very important tool that allows doctors to compare the performance of all home health agencies objectively. They publish objective outcome measures in a way that allows people to compare the performance of different home health agencies. Of all the measures Medicare publicly reports for Ultimate Home Health Care, we beat both state and national averages in 91% of measures (Medicare data updated 3/13/09).

Compared to other Medicare-certified home health agencies in Illinois, our patients are far less likely to require urgent, unplanned medical care (0% of Ultimate patients compared to an Illinois average of 20%). Additionally, they are:

While Medicare certifies us, we've also earned joint commission accreditation. This means we allow the joint commission to conduct routine, unannounced inspections of our services just like hospitals. The commission provides expert, third-party verification of top-quality agencies. It ensures that these establishments have good and effective policies in place, follow their policies, deliver quality care, and continuously strive for improvement in the future.

Meet the President & Director of Nursing

Mabel Osaghae


A devoted Christian and servant leader, Mabel Osaghae, is a RN, BSN and is the President and Director of Nursing of Ultimate Home Health Care (UHHC). Prior to launching UHHC, Mabel's expertise in nursing landed her positions at Crestwood Care Center at Illinois and Ingalls Hospital in the United States and abroad.  As a registered nurse in both Nigeria and the United States, she spent time assisting deliveries and giving post-partum nursing care at First Consultant Medical Center and Ponders End Clinic Maternity in Nigeria.

Mabel found her passion for nursing in her early twenties. After spending several years as a nurse in hospitals, home-care nursing and clinics, Mabel and her husband Vincent decided to take the leap of faith to become entrepreneurs. Since 1998, through Mabel's leadership at UHHC, she has serviced hundreds of clients across the Chicagoland area. 

Mabel is a proud graduate of the School of Nursing in Nigeria where she became the youngest nurse to be certified as both registered nurse and midwife.

In her free time, she enjoys serving in her church and giving back as the co-founder of The Least of These, a microfinance nonprofit organization that offer integrated package of financial and human development programs to the poorest of the poor with a single compelling mission of self-sufficiency.

Mabel is happily married to Professor Vincent Osaghae with three lovely children.
